I understand that for all motor vehicle accidents visits it is my responsibility to pay at the time of service. I understand that I will be expected to acknowledge and sign this patient consent form at each MVA visit. I understand that my financial status will be considered “self pay” for all visits regarding my motor vehicle accident.
I understand that Pioneer Physicians Network will not bill my health or auto insurance for my MVA claims. Pioneer Physicians Network will provide an itemized statement upon request to assist you with billing your automobile insurance.
** EXCEPTION RULE **
There are two health insurance groups that PPN can bill directly for MVA per contract and they are United HealthCare and SummaCare. For these insurances, if the patient has an MVA we bill the health insurance.

    Pioneer Physicians Network is excited to announce that we are now offering TeleVisits, a more convenient appointment option, which allows our patients to have a visit with their medical provider from the comfort of their home or office.  The healow TeleVisits platform allows our patients to have a secure two-way video visit with their provider using a home computer or the healow App from a phone. 

    To schedule a TeleVisit please contact your physician’s office. (Please note that in the event of an emergency you should call 911 to seek immediate medical attention.)

    Ensure you are enabled for our Patient Portal and know your username and password when your appointment is booked. Access the Patient Portal or connect via our healow App HERE.

    To review our list of accepted insurance plans, visit this page. If you are on an Out of Network Medicare Advantage plan, we recommend that you contact an insurance broker to assist you in choosing one of our In Network Medicare Advantage plans.
